I am participating in virtual, in-person, or recorded yoga classes taught by Mindy Eisenberg ("Mindy") and other instructors of Adaptive Yoga Moves Any Body, LLC, a Michigan limited liability company, and Yoga Moves Any Body, a Michigan nonprofit corporation, in which I will receive information about yoga, health, and physical well-being ("Yoga Classes"). I recognize that Yoga Classes require physical exertion which may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved.

I understand that it is my responsibility to consult with a physician prior to and regarding my participation in Yoga Classes. I represent and warrant that I am physically fit and I have no medical condition which would prevent my full participation in Yoga Classes.

In consideration of being permitted to participate in Yoga Classes, I agree to fully assume full responsibility for any risks, injuries, or damages, known or unknown, which I might incur as a result of participating in Yoga Classes.

In further consideration of being permitted to participate in Yoga Classes, I knowingly, voluntarily, and expressly waive any claim I may have against Adaptive Yoga Moves Any Body, LLC, Yoga Moves Any Body, Mindy, and all other instructors of the Yoga Classes for injuries or damages that I may sustain as a result of participating in Yoga Classes and I agree to defend, indemnify, and hold harmless the above parties against these claims.

I understand that if I attend Yoga Classes in person, I may from time-to-time receive hands on adjustments from Mindy or the other instructors of the Yoga Classes in order to correct my position and/or alignment. If I do not want to receive such adjustments, or an adjustment has gone too far, it is my responsibility to advise the instructor at that time.

I give Adaptive Yoga Moves Any Body, LLC and Yoga Moves Any Body the absolute right and permission to publish, copyright, and use pictures and videos of me in which I may be included in whole or in part, composite or retouched in character or form. I give Adaptive Yoga Moves Any Body, LLC and Yoga Moves Any Body permission to use my name and guest's names, photograph or video, likeness or other media of us from events in any promotional materials, publications, or yogamovesanybody.org.
